The episode where Sky goes after the guy that destroyed his father? I have no idea, Im not big on SPD, lol

It was Reflections Parts 1 & 2. Mirloc killed Mr. Tate (Sky's father) who was using early SPD tech based off the Time Force gear (namely Time Force Red). Sky temporarily became SPD B-Squad Red in the second part of the 2-parter to avenge his father with Jack's morpher.

Also, History (along with Wormhole) was a DT/SPD team-up.

Car accident on the I-10 to Sacramento, I believe, and the main cause was that she didn't have her seat belt on and was thrown from the auto when it crashed
